My Blue Heaven should have been named after another Fats Domino hit, ''Ain't That a Shame.'' Steve Martin brings a GoodFellas accent and a closet full of shiny suits to the role of a Mafia type who goes into the witness-protection program and runs amok in a little Midwestern town. Herbert Ross directed, according to the credits, and Rick Moranis and Joan Cusack teamed up to make what should have been the funniest movie couple of the year. Unfortunately, Nora Ephron slapped a boring, predictable, cliché-ridden cement vest of a screenplay on them, and they sank. D
