DEATHS Historian Stephen E. Ambrose, 66, of lung cancer, Oct. 13, in Bay St. Louis, Miss. His best-selling books included 1992's Band of Brothers, on which Tom Hanks and Steven Spielberg's HBO miniseries was based. Ambrose -- who in January refuted plagiarism accusations, admitting only to an occasional failure to use quotation marks -- highlighted the heroism and humanity of American soldiers. Says his sometime coauthor Douglas Brinkley: ''He believed that history should grab you by the lapel.''... Radio personality Horace Logan, 86, whose long-running Louisiana Hayride helped launch the careers of Hank Williams and Johnny Cash, of complications from pancreatitis, Oct. 13, in Victoria, Tex. He is credited with coining the phrase ''Elvis has left the building.''... Actor Dennis Patrick, 84, best known as Dallas banker Vaughn Leland, in a house fire, Oct. 13, in L.A.... Grammy-winning bandleader Ray Conniff, 85, of undisclosed causes, Oct. 12, in Escondido, Calif. His hits included rearrangements of ''Chances Are'' and ''Somewhere My Love.''... Actress Teresa Graves, 54, who appeared on Rowan & Martin's Laugh-In and played the title character in the TV police drama Get Christie Love! in the '70s, in a house fire, Oct. 10, in L.A.
